AmeriBen calls prior authorization precertification. In the self-funded employer plans AmeriBen administers, each plan may require AmeriBen precertification for certain services, and AmeriBen’s Medical Management team reviews the requests. Anthem acquired AmeriBen in the first quarter of 2020, according to Anthem’s second-quarter 2020 results, and AmeriBen now describes itself as a wholly owned subsidiary of Elevance Health, Anthem’s parent company.
AmeriBen’s fax form lists a separate phone and fax number for each employer group on its list, and each employer’s plan document names the services that need precertification. The member’s ID card shows which employer plan applies.

AmeriBen is a third-party administrator (TPA) that runs claims and precertification for employer health plans the employer funds itself. In an AmeriBen FAQ published by IMA (September 2025), AmeriBen says it runs on a claims platform separate from Anthem’s and that Anthem is the network for that plan. A 2024 AmeriBen welcome letter for another employer says AmeriBen uses the Anthem Blue Cross and Blue Shield network, depending on the member’s state. The IMA FAQ also tells members that a provider’s office may not recognize the AmeriBen name and to use the Anthem name instead.
For plans Anthem runs itself, see the Anthem prior authorization guide. Another TPA’s process is in the Meritain Health prior authorization guide.
For questions, AmeriBen’s provider portal sends providers to the number on the back of the member’s ID card, staffed Monday to Friday, 6:00am to 6:00pm Mountain Time.
AmeriBen takes a precertification request in one of three ways:
Incomplete documentation delays the request. AmeriBen gives its decision by call or fax, followed by a mailed letter.
Requests by fax use AmeriBen’s Precertification FAX Request Form (August 2026 version). After the request fields, the form lists each employer group with its phone and fax numbers, plus a fax for inpatient and continued stays. The form says certification is for medical necessity only and does not guarantee payment. Some employers have their own version, such as the State of Connecticut list below.
For a precertification, call the number on the back of the member’s ID card or the employer group’s line on the fax form. AmeriBen Customer Care, at 1-800-786-7930, verifies benefits, eligibility and network status and handles claim issues.
The employer’s plan document, its Summary Plan Description, lists the services that need precertification. Purdue University and the State of Connecticut, both with AmeriBen plans in 2026, show how far those lists differ.
Purdue University’s 2026 standard medical plan, which AmeriBen administers, asks for the precertification call at least 48 hours before services, or within 48 hours after an emergency. When precertification is skipped, the plan may apply a $300 penalty to the resulting out-of-network claims. Bariatric surgery and certain joint and spine surgeries go through Carrum Health for precertification instead, according to Purdue’s 2026 Summary Plan Description.
The State of Connecticut Employer Group and State Partnership Plan have their own AmeriBen precertification list and fax form, effective July 1, 2026. Among other services, it includes inpatient admissions and continued stays, home private duty nursing and certain oncology medications under the medical benefit. Certain other non-oncology specialty medications may need precertification through CarelonRx.

Neither AmeriBen’s fax form nor its portal gives a decision time. The limits below come from 29 CFR 2560.503-1, which binds employer plans under ERISA. A second rule, 45 CFR 147.136, applies the same limits to other group health plans, including self-funded state and local government plans, unless a plan is grandfathered.
| Step | Federal limit |
|---|---|
| Decision on an urgent care request | Within 72 hours |
| Decision on a pre-service request | Within 15 days; the plan may extend once by up to 15 days for matters beyond its control |
| Pre-service request extended for missing information | The claimant has at least 45 days to supply it |
| Filing an appeal after a denial | At least 180 days |
| Decision on a pre-service appeal | Within 30 days, when the plan has one level of appeal |
For how to build the appeal itself, see Silna’s guide to appealing a prior authorization denial.
Silna starts from the member’s ID card and AmeriBen’s employer list to find the right precertification line and fax. It sends the request with the clinical documentation attached and follows up until AmeriBen decides. It keeps each approval’s end date on file, and when a patient’s coverage changes, Silna’s insurance monitoring raises it.
AnswersNow handled double its patient volume without adding staff, according to its case study with Silna. As of October 2026, G2 reviewers rate Silna 4.8 out of 5 across 66 verified reviews.

Yes. Anthem acquired AmeriBen in 2020, and AmeriBen is now a wholly owned subsidiary of Elevance Health, Anthem’s parent company. Many of its plans use the Anthem network, and most precertification requests go to AmeriBen’s own Medical Management team.
It is linked from AmeriBen’s provider portal login page as the Precertification Request Fax Form. Fax it with the clinical documentation to the employer group’s own fax number in the list on the form.
Yes. At provider.myameriben.com, a registered provider can start a precertification request, attach clinical documents and check the request’s status.
No. Each employer’s plan document sets its own list, and AmeriBen’s Medical Management reviews requests against it. Purdue and the State of Connecticut, for example, also send some services to Carrum Health or CarelonRx.
AmeriBen publishes no turnaround time. For most employer plans, federal rules allow 72 hours for an urgent request and 15 days for a pre-service request, extendable once by up to 15 days.
This page is general information about AmeriBen precertification and is not medical, legal or insurance advice. Rules differ by employer plan, so confirm them against the member’s plan document before sending a request.
