Capital Blue Cross is the Blue Cross Blue Shield licensee for 21 counties in Central Pennsylvania and the Lehigh Valley, according to its homepage.

Capital Blue Cross requires preauthorization requests to go through ProAuth. Provider bulletin 2026-10-001 gives these steps:
The Single source preauthorization list shows the services that currently need preauthorization, and Capital Blue Cross can change it at its discretion. Participating providers are responsible for getting the preauthorizations their patients need. To check whether a service needs one, call Provider Preauthorization Services at 800.471.2242, the number the Provider Manual 2026 gives for that check.

A drug request is decided within 24 hours if urgent, or within two business days and no more than 72 hours if not. Medical Assistance (Medicaid) and CHIP managed care plans decide within two business days of receiving all supporting information.
